Teacher End of the Year Assessment (EOYA)
Instructions for the Licensed Teacher: An End of the Year Assessment must include a face-to-face meeting with the child. An individual progress assessment for each student must reflect whether or not the student is making progress commensurate with his/her age and ability in each subject area of the MCOS. The student should demonstrate, during the face-to-face meeting, his/her skill in reading, writing, and math unless the teacher is confident of the child's abilities in these areas based on prior contact with the child or current review of the student's work. Each EOYA letter must include the following student information:
1. the student's name, 2. the student’s age, 3. the parent(s) name(s), 4. the parent(s) address(es), and 5. whether or not the student has a known or suspected disability.
In addition, the EOYA must include the following information for each topic area listed below: 1. Identify the topics the student covered during the course of the school year (i.e. addition, book titles, identification of state capitals); 2. Describe the work samples you reviewed (i.e. worksheets, book reports, experiments performed); 3. Describe how the student made progress in each topic area commensurate with age and ability; and 4. If the student was 12 years old or younger at the time of enrollment, the teacher must include physical education, comprehensive health and fine arts in the assessment.
Prior to completing the assessment, please read the “End of the Year Assessment/VT Licensed Teacher Expectations” in the Vermont Home Study Guidelines document.
