COMMON SCHOLARSHIP APPLICATION PACKET AND GUIDELINES

There are three sections to the Common Scholarship Application Packet and Guidelines: 1. Instructions and Guidelines Page 2. List of Scholarship Opportunities and common application for all local scholarships 3. Supplemental Requirements for each individual scholarship

Attached is a list of the Oak Park High School scholarship opportunities available from local organizations, clubs and individuals. With this single application, you can apply to any scholarship included within this packet.

INSTRUCTIONS:

• Read carefully the description of each scholarship on pages 4-8 and indicate with a checkmark on page 2 the scholarship/s for which you wish to apply. Keep a copy of this list of scholarships for your records.
• Complete the Common Scholarship Application. Do this only once. You will only fill this out once but you must copy this Common Scholarship Application for each of the scholarships to which you apply. Be advised, scholarship committees are looking to see that you can follow directions correctly.
• Fulfill the requirements of the additional supplemental information on pages 4-8 (i.e. additional essays, letters of recommendation) for each scholarship to which you apply.
• YOU must make a copy of each application page (i.e. transcripts, application, personal statement) so that each scholarship committee receives a complete application packet from you.
• Assemble your application and all components into a 9x11 envelope for each scholarship to which you apply. Put the name of each scholarship to which you are applying on the front of each envelope. (If you are applying for three scholarships, you will turn in three envelopes.) Do not put your name on the front of the envelope. Assemble the applications in the following order and insert into each envelope: 1. Common Scholarship Application, copied for each scholarship 2. Personal Resume for activities participated in during grades 9-12 including school activities, all extracurricular activities, volunteer experience, work experience and leadership roles 3. Financial need explanation on a separate page, if applicable, copied for each scholarship application 4. Personal Statement, found on page 3 of the application, copied for each scholarship application 5. Current copy of unofficial transcript, (order only one copy and photocopy it for each scholarship application) 6. Additional Supplemental requirements requested by individual scholarships 7. Letters of recommendation, if requested
• DO NOT use a cover letter or staples (paper clips may be used)
• Applications and completed packets must be received in the College & Career Center by 12:30 p.m. sharp! Wednesday, April 16. No exceptions will be made.
• If you have any questions regarding the scholarships or the Common Scholarship Application, see Ms. Hawkins in the College & Career Center for assistance. Scholarships will be awarded at the Senior Awards Night on Thursday, May 29.
• A COMPLETE packet must be assembled for EACH Scholarship to which you apply!!!!

2014 Common Scholarship Application Opportunities
SUPPLEMENTAL REQUIREMENTS FOR EACH SCHOLARSHIP
To the Student: Complete the supplemental requirements for EACH scholarship that you wish to apply for (i.e. additional essay) in addition to the Common Scholarship Application. See instruction page for information on how to assemble each scholarship application packet.
Turn in the completed scholarship packets to the College & Career Center, C-6, by 12:30 p.m. sharp! on Wednesday, April 16. Keep this list of scholarships for your file. Do not include it in your packet.
OPHS Scholarships
Oak Park Classified Association Scholarship
Student Profile: Graduating senior planning on attending a 2, 4 year college or university or trade school who has been involved in school activities and extra curricular activities outside OPHS. Minimum 2.0 GPA
Supplemental Requirement: • Submit 100-200 word statement describing education and career goals. Describe an achievement you are especially proud of involving school or community and explain why you want to be considered for this scholarship. • Two letters of recommendation required.
Award: The number of scholarships and the amount of each award changes yearly.
Oak Park High School Athletic Booster Club Scholarship
Student Profile: Graduating senior who plans to attend a community college, technical school or four year college or university either on a full time or part time basis. Minimum 2.5 GPA
Supplemental Requirement: • On a separate piece of paper please submit information on: o All sports you participated in at OPHS and awards/recognition received (include all grade levels) o Sports clubs/teams not affiliated with OPHS and awards/recognition received (all grade levels) • Submit a typed, double spaced, personal essay (no longer than 2 pages) which includes the information below. This essay is an important part of the selection process and will be kept in strict confidence. o Your goals for the future. This is an opportunity for you to share your hopes and goals with the Scholarship Committee. o How has your athletic experience at Oak Park High School prepared you for the future? o Include any information you believe the scholarship Committee should have. For example, family income, estimated expenses and/or circumstances that may affect the affordability of a post secondary education should be included.
Award: The number of scholarships and the amount of each award changes yearly.
Oak Park High School Choir Booster Scholarship
Student Profile: College bound senior who has participated with the OPHS choir sometime during their high school years. Applicants must have plans to continue studying in the area of Performing Arts, with special attention to students majoring in voice, choral general and/or musical theatre.
Supplemental Requirement: • Submit an essay (double spaced essay, approx.. 2 pages) based on the following questions: o What has being a member of the choir program at Oak Park High School meant to you and how long have you been a member? o What are your future plans for college & career? o Describe your most meaningful/favorite experiences while being a member of the OPHS Choir? o Explain why receiving this scholarship would be important to you. o Explain why you feel that you are deserving of this scholarship. o Explain how you feel that you have shown leadership while being in the choir, especially as a senior. o List as an appendix: all awards / solos / officer positions that you have received as a member of this OPHS choir.
Award: Will be allotted at the discretion of the choir scholarship committee, which will consist of the choir director and at least three non-senior choir parents.

Oak Park High School Instrumental Music Senior Scholarship Award
Student Profile: Must be a college bound graduating senior who has been involved in the OPHS instrumental music program sometime during their years at OPHS. In good standing with a 2.0 GPA or higher and must have plans to continue studying in the area of Performing Arts with special attention to students majoring in music.
Supplemental Requirement: • Applicants must submit an essay (double spaced, approx. 2 pages) based on the following questions: o What has being a member of the Instrumental Music program at Oak Park High School meant to you an how long have you been a member? o What are your future plans for college and career? o Describe your most meaningful/favorite experiences while being a member of the OPHS Instrumental Music program. o Explain why receiving this scholarship would be important to you. o Explain why you feel that you are deserving of this scholarship. o Explain how you feel that you have shown leadership while being in the Instrumental Music program, especially as a senior. o List as an appendix: all awards / solos / officer positions that you have received as a member of the OPHS Instrumental Music program.
Award: The number of scholarships to be awarded at discretion of the Instrumental Music Director and at least three non-senior music class parents.
Oak Park High School Parent Faculty Club Scholarships
Student Profile: Graduating senior with 2.5 GPA and above and must be planning to attend a community college, technical school, or four year college. Recipients will be selected from all of the following GPA ranges: 2.5-2.99, 3.0-3.49, 3.5-4.0.
Special Requirements: • Personal essay or short constructed responses to six questions: An essay in response to one of the following topics (no more than 2 pages – typed, double spaced total, with Student id# and no name in the top right hand corner of each page). a. A defining moment, person or event in your life. b. Where will you be in 10 years? c. What have you learned from your extra curricular experiences? d. Explain why receiving this scholarship would be important or beneficial to you. OR Submit short, constructed responses to six of the following questions (no more than two pages typed, double spaced total, with Student ID # and no name in top right corner of each page): A. Describe your best traits and strengths. B. Describe your weaknesses and what you have done to overcome them. C. In what way are you different from the person you were when you started high school? D. Describe a mistake you made and what you learned from it. E. How are you different from your family? F. Who is your role model and why? G. If you could change one thing at OPHS, what would it be and why? H. What would you like to do with your life?
Award: Letters of Recognition and $500 each award
Oak Park Performing Arts Alliance, In memory of Rick Rhodes
The Oak Park Performing Arts Alliance awards annual scholarships to graduating seniors who have participated in the Theatre Arts Program at Oak Park High School. These awards are given in memory of Rick Rhodes, a talented and accomplished musician and songwriter who was the creative force behind the performing arts program at Oak Park High School. He helped create the theatre arts program here and provided a level of professionalism that has influenced many students in the arts program.
Student Profile: The applicant must have participated in the theatre arts program at OPHS (on stage or crew) and have a minimum 2.5 GPA.
Supplemental Requirements: • The applicant must include a list of any OPHS productions as well as outside OPHS productions participated in, as well as any performing arts awards. • The applicant must complete the application’s record of extra-curricular activities • The applicant must attach a typed, double-spaced, personal essay no longer than 2 pages, which includes information from 3 of the 4 areas below. This essay will be an important part of the selection process. This essay should paint a clear picture of who you are. Essays will be read by a panel of OPPAA members, as well as outside community members. Any information you provide will be kept strictly confidential. o How has your theatre experience at Oak Park High School prepared you for your future endeavors? o Your goals for the future. This is an opportunity for you to share your hopes and goals with the Scholarship Committee. o What has being a part of the Performing Arts program meant to you? o Describe your most meaningful performance/project in theatre at OPHS.
Award: Three awards: $1500, $1000 and $1000.

Oak Park Teachers Association Scholarship Student Profile:
Graduating senior who is planning to attend a 2- or 4 year college/university or trade/technical school. Student must have a 2.5 minimum GPA at OPHS or OVHS and be involved in school and/or community activities. Supplemental Requirements:
• Submit a 100-200 word statement explaining school/community/volunteer service and how this impacted your future
decisions regarding college/university/trade/tech school major/career opportunities.
• Submit two letters of recommendation from two current faculty members. Submit a third letter of recommendation
from an adult in the community in connection with your community/volunteer service. Award:
$1000
Oak Park High School Memorial Scholarships

Marie Panec Memorial Scholarship for Integrity and Advocacy

Student Profile:

Must be a graduating senior at Oak Park Unified School District and will be awarded in recognition of that student’s exemplary integrity,

compassion and advocacy for the interests of others. Although academic achievement may be considered in determining the award recipient,

it shall be accorded less weight than the key criteria. (Dr. Marie Panec, a trustee of the Oak Park Unified School District Board of Education, a

Moorpark College professor and a community volunteer, focused on improving the community in which she lived through education.)

Supplemental Requirements:

• One or more Letters of Recommendation are optional, but may strengthen your application.

• A 300 word maximum personal statement, explaining why you feel you are most qualified for this scholarship. Examples of subjects include,

but are not limited to: (1) discussing a time in which you advocated for the interest of another, or (2) discussing what ideals are important to

you and how your pursuit of these ideals exemplifies integrity, compassion or advocacy.

Brian M. Wolverton Award for Exemplary Citizenship In memory of Brian M. Wolverton

Student Profile:

Must be a graduating senior at OPHS, Oak View or OPIS that possesses exemplary citizenship and character. Applicants must have a

minimum of a 2.0 GPA.

Supplemental Requirements:

• Two Letters of Recommendation that attests to your character and exemplary citizenship from a coach, teacher, counselor or employer.

• A 250 word essay on “What does good citizenship mean to you?”

Award:

Anton Sokolow-Nikolic Golf Scholarship

Student Profile:

Senior must be a member of the OPHS golf team, have a minimum of 2.8 GPA, demonstrate a strong

commitment to the game of golf.

Supplemental Requirements:

• Submit a 150 word statement, describing your commitment to golf, your future plans, including your education and career goals

as well as listing the reasons why you would like to be considered for this award.

• Statement of financial need

• Three letters of recommendation, attesting to your reliability, integrity and dedication: one from a golf professional--not the Oak

Park High School golf coach; one from a teacher or school official, and one from an employer or supervisor (volunteer or paid).

Oak Park Elementary School Scholarships

Oak Hills Elementary School Scholarship
Student Profile: Graduating Oak Park High School senior who culminated from Oak Hills Elementary School and plans to attend a community college or a 4year college or university. Minimum 3.0 GPA.
Supplemental Requirements:
• Submit a typed, double spaced essay that describes in 500 words or less an experience or teacher from Oak Hills Elementary School that helped shape you into the person you are today.
• Submit on a separate sheet of paper the grade levels you attended at Oak Hills Elementary School. Award: $500

Local/Community Scholarships
Agoura Youth Basketball Association (AYBA)
Student Profile: Graduating senior must have played, coached, worked, or volunteered in some other capacity with AYBA programs for at least 2 seasons.
Supplemental Requirements: • One (1) Reference Letter • Personal Statement – Include “How participation in AYBA has helped you.”
Award: Two awards to be offered; $500 each
Baha’i of Oak Park Scholarship
Student Profile: College bound senior who has a minimum 3.0 GPA and is headed to a 2 or 4 year college or technical school. Senior needs extensive community, religious and/or school extra curricular activity. There will be a strong emphasis on community service, scholastic achievements, goals and moral values.
Supplemental Requirements: • Essay - one page maximum 250 words, should describe in detail your activities regarding voluntary service and what you have learned from these experiences.
Award: $250
Westlake Village Rotary Club
Student Profile: College bound senior. Students will be evaluated based upon scholastic records, school activities, financial need, community service and employment experience.
Supplemental Requirements: • On the top of your supplemental essay, please include the following information: o Name, complete address and zip, and phone number o Birth date and social security number o Father and Mother’s occupation or previous occupations. o Brothers and sisters, please list names and ages o Approximate Family Income: under $50,000 $50 - $75,000 $75,000 - $100,000 over $100,000 o List any awards or scholarships you have received o List hobbies or interests. o List all work experience, length of employment and job description. o List your personal goals. • Essay – Attach an autobiography of not more than 500 words. Be sure to indicate your personal goals and why you feel you should receive this scholarship.

Westlake Women’s Club
Student Profile: Financial need based scholarship for male or female graduating senior with a 3.5 GPA or above. Participation in school and/or community activities, required. (There are two scholarships available, including one districtwide)
Supplemental Requirements: • Signed essay defining your goals – no more than 200 words • Financial Statements of parents or guardians and a copy of their current U.S. Tax return may be required • Two (2) signed letters of recommendation: one from a high school teacher and one from your counselor with name and phone number of the counselor
